This portal is divided into 4 main sections:

The initial onboarding phase where client information, intake forms, consent documents, scheduling, and preliminary details are collected and organized.

The active support and treatment phase where therapy sessions, progress tracking, wellness resources, and ongoing care coordination take place.

A clinical review phase focused on understanding the client’s needs through evaluations, observations, session notes, and treatment recommendations.

The closing or continuation phase where final notes, follow-up recommendations, discharge planning, and long-term wellness support are managed.

To help us begin your care process smoothly, we’ve outlined a few important onboarding tasks below. Each task contains additional instructions and information, so please click and open each item to review the details carefully before completing it.
Once you have fully completed a task, please update the Status property and select “Submit” to notify our team for review.

Clear and supportive communication is an important part of the care experience, and we’ve made it easy for you to stay connected with our team throughout your journey.
Your primary point of contact will be your assigned care provider or support coordinator. However, you’re always welcome to use the Sessions section to schedule appointments, follow-ups, or check-in sessions whenever needed.
We’ve also added a Chat Feature right on the home page. Feel free to send a message anytime you have a question, need clarification, would like additional support, or wish to request a follow-up from our team.
